Cardiolipin is a fat that lines the inside of mitochondria, the parts of a cell that make its energy. SS-31 is designed to latch onto it, which is why it turns up wherever mitochondria fail.
A 2026 drug profile reports that elamipretide won accelerated approval in the United States in September 2025, for use in Barth syndrome, aimed at muscle strength. Barth syndrome is an inherited condition that harms cardiolipin. That approval covers one rare disease and one medicine. This research vial is not that medicine and has no approval for use in people.

Which SS-31 trials does this page name?
A 2025 review gathered the animal work on elamipretide across the heart, kidney, brain and muscles, together with its human trials.
In a 2020 crossover trial, 30 adults who had a muscle disease rooted in mitochondria each had both elamipretide and a placebo, and the trial measured a walking test and how tired they felt. On the walking test it found no clear gain.
Barth syndrome was the focus of a 2021 phase 2/3 trial with 12 patients. Its first part was randomised, double-blind and placebo-controlled, with a crossover design, so each patient had both elamipretide and a placebo. Its main measures were a 6-minute walk test and a Barth syndrome symptom scale, alongside knee muscle strength, the patients' own view of their symptoms and some heart measures. In that controlled part, neither main goal was met. A later open part, which 10 of the 12 joined, gave elamipretide to everyone, with no placebo group to compare against. Across these trials the numbers of patients are small.
